It's nice that you want to participate in the Preventive Medical Examination (PMO) at Ardagh Glass Packaging in Dongen. Complete the sections below to create an account. Have you previously participated in the PMO via Active Living and do you already have an account? After entering your data, you will receive an instruction by email.

Entering your mobile number is often not mandatory. When you enter your mobile number, we will send you a reminder text message 24 hours prior to your appointment. From 45 years and older you are also eligible for an ECG (resting ECG). If the doctor discovers an obvious abnormality, the doctor will contact you on this number. In this case, we request that you enter your mobile number.

We also ask you to provide some information about your work, so that we can prepare an (anonymous) report for Ardagh Glass Packaging.

Under 'Location' you enter your work location, under 'Department' your team (Operations shift A/B/C/D/E) in which you work. You must also indicate that you want to participate in the PMO, so that Ardagh Glass Packaging can schedule an appointment for your PMO.

Finally, it is important to mention that we always make an anonymous report, which can never be traced back to you as an individual. This means that when preparing the report, we only report on groups larger than 15 people. If you hold a position that is performed by fewer than 15 people, job groups will be combined before we report. This means that a result can never be traced back to a single position or person.
